Tirzepatide achieves robust weight loss (15–25% of body weight), significant glycemic improvement, and substantive fat mass reduction in clinical trials (SURPASS, SURMOUNT). Systematic reviews demonstrate L-carnitine further supports fat oxidation, metabolic rate, and preserves muscle mass with weight loss. Common adverse effects: GI symptoms (nausea, vomiting, constipation, diarrhea), mild injection site irritation, fishy odor (from L-carnitine), appetite suppression. Serious risks: pancreatitis, gallstones, hypersensitivity, and potential contraindication in patients with MEN2 or medullary thyroid cancer history (tirzepatide specific); close monitoring highly recommended.

CarniTide’s use is validated only in adult populations; not recommended in pediatric, pregnant, or lactating women. Dose modification and enhanced monitoring are required in renal, hepatic, or complex metabolic states.
