Compounded Semaglutide+L-Carnitine (CarniSema™)

Compounded Semaglutide+L-Carnitine (CarniSema™) Injection is a specialty compounded formula containing 2.5 mg/mL semaglutide and 100 mg/mL L-carnitine in multidose vials, dispensed exclusively by Lake Hills Pharmacy and Nimbus Healthcare. This product is designed for weight management and metabolic health, leveraging semaglutide's appetite and glucose effects and L-carnitine's support of mitochondrial fatty acid oxidation.

Semaglutide acts as a GLP-1 receptor agonist to stimulate insulin secretion, suppress glucagon, slow gastric emptying, and induce central satiety—lowering appetite and improving glycemic control. L-carnitine supports mitochondrial import and β-oxidation of fatty acids, promoting enhanced fat metabolism and cellular energy availability. Together these act synergistically for greater fat loss, metabolic improvement, and glycemic control.

Common effects: injection site redness, swelling, mild discomfort; gastrointestinal symptoms (nausea, diarrhea, constipation) ; appetite suppression, headache, dizziness, taste changes, fatigue. Rare but serious: allergy, hypersensitivity, or pancreatitis. Most adverse effects are mild to moderate, largely reflecting semaglutide’s profile.

Contraindicated with allergy to semaglutide, L-carnitine, or excipients; medullary thyroid carcinoma/MEN2 history; severe renal impairment (for carnitine); pregnancy, lactation, or children unless medically necessary. Precautions for GI disease, pancreatitis history, or co-use with hypoglycemics. Monitor for weight, glucose, appetite, injection tolerance, allergenic or GI symptoms.

Administer via subcutaneous or intramuscular injection using appropriate technique. The typical starting dose is semaglutide 0.25–2.5 mg per week and L-carnitine 100–1000 mg per week (1–10 mL of 100 mg/mL solution), titrated as tolerated and prescribed. Injections are performed weekly or biweekly as indicated by the clinical protocol. Gradually increase the semaglutide dose to minimize GI side effects. Do not exceed L-carnitine 1000 mg (10 mL) per injection/week. Maximum semaglutide dose is 2.5 mg per week.
